Who is eligible for Services?
Children who are Sanilac County residents, between 0 - 17 years of age, 并且有严重情绪障碍和/或发育障碍的症状.
Serious Emotional Disturbance
A diagnosable mental, behavioral or emotional disorder affecting a minor, resulting in functional impairment that significantly interferes with, or limits the minor's role or functioning in family, school or community activities.
Developmental Disability
A disability due to a mental and/or physical impairment, 发生在22岁之前，导致以下三个或更多领域的严重功能障碍, learning, mobility, disability due to a mental and/or physical impairment, ability to make decisions about life, ability to live independently, and ability to hold a job.

Services include the following:

Assessment and Service Planning: 初步和定期评估，可能包括一系列专业和心理评估，以确定适当的诊断和服务资格, to recommend a course of treatment, and to develop a plan of service using the person centered planning approach.

Counseling: Sanilac CMH offers individual, group and family therapy. The primary goal of this service is to help reduce problem behaviors, improve self-control, increase motivation, improve emotional adjustment and improved day to day functioning.

Services are provided at the Administration Building.
Wraparound Services: The goal of Wraparound services is family preservation and independence, so families are able to operate successfully with minimal agency intervention. 高度个性化的规划过程由专门的案例管理人员执行，他们负责协调以下方面的规划, and delivery of, 包罗式服务和儿童医疗上必需的非工作人员附带项目. Wraparound utilizes a Child and Family Team determined by the family, often representing multiple agencies, and informal supports. Children must meet two or more of the following: have involvement in multiple systems; be at risk of/currently in out of home placements; received mental health services with minimal improvement; have risk factors that exceed capacity of traditional community based options; be one of multiple children in a family served by numerous providers without meeting desired outcomes.
Wraparound services include the following:

- Case Management: Assessment, planning, linkage, advocacy, 协调和监测，协助个人获得所需的保健和牙科服务, financial assistance, housing, employment, education, social services, 以及通过以人为本的规划过程开发的其他服务和自然支持.

Family Support Services include the following:

- Supports Coordination: Available for children who have been diagnosed with a Developmental Disability. Services include giving assurance of person centered planning, linking to, coordination with, and follow up of, 倡导和/或监测精神卫生和社区服务和支助. 该方案的主要目标是让儿童留在家中，享受限制最少的服务.
